SCAN Classic & Connections

SCAN Health Plan Arizona offers the following Medicare Advantage plan for residents in certain Arizona counties. To enroll in the SCAN Classic (HMO), you must meet the following criteria:

• You must be a resident of Maricopa or Pima counties
• You must be eligible for Medicare Part A and Medicare Part B plans

Persons diagnosed with End-Stage Renal Disease may not elect coverage under this plan.

• In-network doctor and hospital choice
• Coverage throughout the US when traveling
• Unlimited inpatient hospitalization for Medicare-covered hospitalizations
• 190-day Inpatient Mental Health Care
• Skilled Nursing Facility care
• Network doctor’s office visits ($10 copay)
• Network urgent care visits ($30 copay)
• Network specialist care visits ($35 copay)
• Emergency room visits ($50 copay)
• Drugs covered under Medicare Part B (20% of cost)
• Formulary drugs covered under Medicare Part D
• Five-tier pharmacy copays for generic prescription drugs
• Vision coverage
• Preventive care services

Medicare Advantage SCAN Connections (HMO SNP)

The SCAN Health Plan is a Dual Eligible/Special Needs Medicare Advantage plan designed to meet the needs of certain Medicare-eligible beneficiaries in Arizona. Not all Medicare beneficiaries in the plan’s service area may enroll. To enroll in the SCAN Connections (HMO SNP), you must meet ALL of the following eligibility requirements:

• You must be a Maricopa County resident
• You must be Medicaid (ALTCS) eligible
• You must be Medicare (Part A and Part B) eligible
